You can now download the following forms. After downloading, fill in the forms and mail them to us. If you can't find the form that you want, or if you have further enquiries, please call our Customer Service Centre during business hours at 03-2095 7612 / 7613. Our customer service staff will be pleased to assist you. 
  • Request For Change Form (64.3KB) - For change of payment mode, policyowner's information, etc.
  • Absolute Assignment Form (56.0KB) - For assignment of policy to financial institution, company, association, person, etc as a collateral security for a loan.  
  • Health Declaration Form (72.4KB) - For reinstatement of a lapse policy. Terms and Conditions apply.

PROCEDURE ON CHANGE OF PAYMENT MODE :

The payment modes available are monthly, quarterly, semi-annually and annually. This is however applicable to Ordinary Life policies only. Policyowner may request to change the payment mode at anytime subject to the following rules and requirements:-

Annual At policy anniversary date (PA) modal premium for current mode/0.087
Semi-annual At PA or 6 months from the PA (modal premium for current mode/0.087)*0.52
Quarterly At PA or 3 months or 6 months from the PA (modal premium for current mode/0.087)*0.261

** Must arrange for payment by standing instructions or Maybank autodebit instruction on policyowner’s account.

  2. Please fill in the following particulars:
    • Policy No.: Your policy number
    • Insured: Name of the insured
    • Details of amendment/change requested: To change my payment mode from (existing payment mode) to (new payment mode)
    • Place and Date: The place and date when this form is signed
    • Signature of Insured/Policyowner: Signature and details of the Policyowner. The signature must be the same as our records and must be duly witnessed by a disinterested party.
    • Signature of witness: Signature and details of the witness.
  3. Send the completed form together with payment, if any, to

Note:

You may pay your premiums :

  1. directly to our Head Office or any of our branch offices, or
  2. by Standing Instructions with your bank or by Autodebit (arranged with Maybank), or
  3. through the Personal Internet banking system of MayBank Maybank2u or Public Bank PBeBank
